1. The date of the event
2. Type of event (Class Reunion, Birthday, or Wedding, etc.)
3. Theme of the event (Disco, or Sock Hop, etc.)
4. Location of the event (City and State)
5. Is the event an Indoor or Outdoor event?
6. Number of invited Guests
7. The time your guests are invited to Arrive at the event
9. Name of Venue where the event is taking place
10. Name and telephone number of the Venue Coordinator
11. Your first and last Name
12. Contact information (telephone number and/or cell phone number and best time to reach you)
13. For Weddings ~ First and last Name of the Bride and the Groom
14. For Weddings ~ Do you need Ceremony Music played by us?
15. For Weddings ~ Will your Ceremony and Reception take place at the same location? Include the Locations of each
